Depending on the services and nature of your appointment, our fees vary. Contact us for specific pricing on 07 3088 2323, or see our terms.
We offer bulk-billed services through Medicare, however, if you’ve already used your five Medicare-covered sessions, a standard fee will also apply.
Please note, we require credit card details to secure your appointment. Your card is only ever charged at the end of the appointment for private appointments, or if you fail to attend the appointment and a cancellation fee applies, or if a bulk billed service isn’t honoured by Medicare (https://hla.au/terms).

If you want a bulk-billed appointment, you will need either a GP Chronic Condition Management Plan (GPCCMP) or Department of Veteran Affairs referral. These plans allow your appointment to be covered by Medicare and DVA respectively.
For private appointments, a referral is not required. Just ring to make an appointment on (07) 3088 2323 or book online.
For NDIS appointments no referral is required. An intake form needs to be filled in though to make a booking. -
Yes, some of our clinicians offer bulk billed appointments depending on your eligibility. Please contact us to determine whether you are able to access this.
We offer bulk-billed consultations for patients with a valid Medicare card, Department of Veterans' Affairs (DVA) cardholders and NDIS participants where we have received the required referral documentation. If we have not received the correct referral documentation or service agreement we are not able to bulk bill the session and you will still be able to have the session but charged privately.
Medicare:
We offer bulk-billed consultations for eligible clients through Medicare. To access Medicare-funded allied health services (such as dietetics, exercise physiology, or diabetes education), you must have a valid GP Chronic Condition Management Plan (GPCCMP) prepared by your general practitioner.
This plan gives you access to up to five Medicare-subsidised allied health appointments per calendar year.
Please note:
Not all clinicians offer bulk billing. Some senior clinicians may charge a gap fee.
Your GP must send the referral before your appointment.

DVA:
We offer bulk-billed consultations for Department of Veterans’ Affairs (DVA) clients.
If you hold a valid DVA Gold Card, you are eligible for fully covered services for all clinically necessary treatment, including allied health services such as dietetics, exercise physiology, and diabetes education.
If you have a DVA White Card, you may also be eligible for bulk-billed services, but only for treatment related to your accepted service-related conditions.
To access these services, you will need a valid referral from your GP - typically a D904 referral form, which remains valid for 12 months or a set number of sessions, depending on the service.

NDIS:
We are a registered NDIS service provider. We provide services for NDIS participants who are either plan-managed, NDIA managed or self-managed. Fees apply in accordance with the NDIS pricing arrangements. Please ensure your NDIS details are provided when booking your appointment.
We support a range of NDIS-funded allied health services, including Dietitians, Exercise Physiologists, Diabetes Educators and allied health assistants. Services are billed in line with the current NDIS Pricing Arrangements and Price Limits.
Please note:
A signed NDIS Service Agreement is required before services can begin.
Self-managed participants pay at the time of service and can claim reimbursement through the NDIS portal.
Plan-managed participants will have invoices sent directly to their plan manager.
NDIA managed participants will have invoices claimed directly from the NDIS.

For clients who are not eligible for Medicare, DVA, or NDIS funding, or who choose to pay privately, we offer private consultations.

A GP Chronic Condition Management Plan (GPCCMP) is a plan developed by your general practitioner (GP) to help manage your chronic health condition. It allows you to access Medicare rebates for certain allied health services, including Dietitians, Diabetes Educators and Exercise Physiologists, through a limited number of subsidised appointments.
You might receive a GPCCMP if you have conditions such as diabetes, heart disease, arthritis, obesity, or other long-term health issues that require ongoing care from allied health professionals.
This plan can make you eligible for up to five Medicare-subsidised allied health appointments per calendar year to support your condition.
Having a GPCCMP means you can receive bulk-billed or reduced-cost services as part of your ongoing healthcare, making it easier to manage your condition with professional support.
Only some clinicians offer bulk-billed services under a GPCCMP. Please refer to our booking conditions for details.

As part of recent changes to Medicare referral processing, we now require all clients to provide credit card details at the time of booking, even if your appointment is eligible for bulk billing.
This change helps ensure we can continue offering bulk-billed services while maintaining fairness and sustainability. While we aim to bulk bill eligible appointments through Medicare, there has been an increase in unpaid or rejected Medicare claims. If a claim is unsuccessful, we may charge you directly for the standard Medicare rebate amount (currently $61.80 per session).
Your card will only be charged if a Medicare payment is declined or not processed.
This process helps protect the availability of bulk-billing services for all patients and allows us to continue providing accessible, high-quality care.

Yes. For Healthy Lifestyles Australia clients, appointments that are not attended or cancelled with less than 24 hours' notice will incur a $39.95 cancellation fee.
If we already have your credit card details on file, the cancellation fee will be charged automatically.

To ensure you get the most out of your consultation, please bring the following:
Food Diary: For your initial consultation with the dietitian we suggest bringing along a food diary of up to three days. This should include breakfast, lunch, dinner any tea, coffee, water or supplements. Document Food Diary
Relevant Paperwork: Such as a current list of any medications you're taking or recent medical reports (including any relevant test results, such as blood work).
Referral Letter: If applicable, bring your GP Chronic Condition Management Plan (GPCCMP) referral.
NDIS Plan Details: For NDIS participants, bring your plan details and any relevant documentation.
Questions or Concerns: Any specific topics or questions you'd like to discuss.

To make the most of your session, please bring:
Exercise Pre-Screening Questionnaire: Please complete the Healthy Lifestyles Australia Exercise Pre-Screening Questionnaire before your appointment if you are participating in group exercise programs, fitness challenges, exercise testing, or exercise as part of your consultation. This helps us assess all factors to mitigate risks and promote your safety during exercise.
Exercise Participation Waiver: Please complete the exercise participation waiver if you have been asked to and are participating in a Healthy Lifestyles Australia led event, program, or challenge.
Appropriate Clothing and Footwear: Comfortable clothes and shoes suitable for physical activity as you may be asked to do some movements or stretches.
Relevant Paperwork: Such as a any recent medical or diagnostic reports related to your condition or injury.
Referral Letter: If you have a GP Chronic Condition Management Plan (GPCCMP) or other relevant referral, please bring it with you.
NDIS Plan Details: If you’re an NDIS participant, please bring your plan and any related documentation.
Questions or Goals: Any specific questions, concerns, or goals you want to address during your appointment.
Bringing these items helps your exercise physiologist tailor your program safely and effectively.

Medications List: A current list of all medications and supplements you are taking.
Blood Glucose Records: Any recent blood sugar readings, logs from your glucometer, or data from diabetes apps/devices.
Medical Reports: Relevant pathology results or recent test results, if available.
Referral Letter: If you are using a Medicare-funded appointment, please bring your GP Chronic Condition Management Plan (GPCCMP) referral.
NDIS Plan Details: If you're an NDIS participant, please bring your plan and any related documents.
