Booking Terms and Conditions

Cancellation Policy:

For Healthy Lifestyles Australia clients, appointments not attended or cancelled with less than 24hrs notice will incur a $39.95 cancellation fee. For NDIS related service delivery please refer to the linked page for NDIS cancellation fee policy. If we already have your credit card details on file we will charge the cancellation fee.

Please note Healthy Lifestyles Australia also provides reception services for Mental Edge Psychiatry located in our Hervey Bay office. If you are booked in with the psychiatrist please refer to Dr Beau’s website for further terms and conditions specific to his services and cancellation fees. https://www.mentaledge.net.au/consultation-fees

Selecting Appointment Type for Online Bookings, Over the Phone Appointments and all other appointments:

If you select a bulk billed appointment type (Medicare or DVA) you are required to upload your referral (online bookings). If booking over the phone please note we require a valid copy of the required referral to bulk bill. In both instances come the appointment time if the correct referral type is not provided or we don't have it on our system you will be required to pay the full consultation amount. If you have a valid referral but have utilised all you allocated Medicare bulk billed appointments for the year and our claim is not honoured by Medicare or DVA you will be liable for payment.

For online bookings please select "Initial" appointment type if it is the first time seeing one of our professionals or if it has been longer than 1 year since your last consultation, otherwise please select "Review" appointment.

If you book an online appointment you will be asked to pay for your appointment prior to the appointment date for us to successfully confirm that time for you. You will be contacted from our reception team prior to the appointment date to make payment arrangements.

Entering Credit Card Details on the Booking Portal:

We will only charge the credit card details provided on the online booking portal should you fail to attend your appointment, or fail to inform us within 24hours of the appointment time that you are unable to attend. Please refer to the cancellation policy above to determine associated fee. If you attend your appointment payment of your consultation fee will occur at the time of your appointment. If you are seeing an allied health professional at one of our head centres HICAPs processing will be available to you at your time of appointment.

Why do I need to enter credit card details on the online booking portal if I have a bulk billed referral type?

If you fail to attend your bulk billed appointment we are unable to claim any payment for that appointment. As per the above cancellation policy you will be liable to pay the (late cancellation/failure to attend) fee so we can cover the costs of providing services.

Initial 1 hour consultations = $160
Review 30min consultations = $80
Medicare Allied Health Items = Bulk Billed (Valid referral required)*
Department of Veteran Affairs = Bulk Billed (Valid referral required)*

Exercise Physiologists

Initial 1 hour consultations = $176
Review 30min consultations = $88
Medicare Allied Health Items = Bulk Billed (Valid referral required)*
Department of Veteran Affairs = Bulk Billed (Valid referral required)*

*Please note that not all Healthy Lifestyles Australia clinicians bulk bill and directors and other senior staff members charge higher hourly rates. To confirm the rate of your consultation please contact us on (07) 3088 2323.

*If you have already utilised all your bulk billed sessions for the year and we do not receive the fee from Medicare or DVA you are liable to pay the amount owing that otherwise should have been paid by Medicare or DVA. If you are unsure to how many allied health visits you have already claimed for the year feel free to contact Medicare to discuss eligibility. You are able to access 5 allied health visits under a Medicare Chronic Condition Management plan per calendar year.
